Forms available for Downloading

A Surety is required that all erosion and sediment controls are properly installed and maintained during construction and to assure that the site is stabilized at the completion of the project.

§  Performance Bond - from either an insurance or bonding company licensed to do business in the State of Alabama

§   Irrevocable Standby Letter of Credit - available from a Credit Union or Bank doing business in Jefferson County, Alabama containing automatic renewal language. Letter of Credit must be on the bank's letter head.

§   Certificate of Deposit - from a bank located in Jefferson County. CD must meet SWMA requirements. Currently, it’s unknown if any banks now prepare CD’s that meets SWMA requirements
