See the Erosion and Sedimentation Control Ordinance, Sections 5.01 and 5.02 for ALL applicable requirements (see attached). This is a checklist of minimum requirements.
All proposed site improvement must be shown on the plan, including, but not limited to: buildings, parking, (curb and gutter or otherwise should be apparent from the grading plan submitted), storm water pipes, storm water inlets, storm water flumes, proposed drainage ditches, entrance/exit drives, etc. (Also, see the ordinance, section 5.02)
Grading Plan sealed and signed by a registered professional engineer, architect, landscape architect.
Erosion and sediment control measures must be located on the plan and details provided. Some examples are:

Check dams along areas of concentrated flow(50 ft. spacing maximum between check dams) and provide an appropriate detail.

Construction entrance/exit located on the plan and a detail provided (2-3 minimum diameter stone, 12wide and 50 long).

Stacked Hay bales. Locate on the plans and provide a detail for installation.

Silt fence-specify if Class I or Class II is to be used, specify if silt fence and hay bales are to be used simultaneously. Locate on the plans and provide an appropriate detail.

Storm Water Outlet Erosion Control (pipe outlets, flume outlet, etc.).

Storm Water Inlet Sediment Control (pipe inlets, headwalls, yard inlets, fume inlet, etc.).

Rip-Rap in areas where long-term erosion may be a problem.

Erosion control blankets/mats as appropriate for slope, flow volume and flow velocity.

Sediment ponds as appropriate for anticipated site drainage and possible sedimentation.

Etcetera-other measures as determined by the qualified credentialed professional and appropriate for the specific site.
Required Notes (directly from the Ordinance, see the attachment) in addition to any notes deemed appropriate by the qualified credentialed professional.
The Amount of Disturbed Area (in AC) must be noted on the plans.
The Proposed Project Start and Complete Dates must be noted on the plans.
Erosion /Sediment Control Plan signed (sealed if appropriate) by a qualified credentialed professional
